  Please choose only ONE of the following workshop discounts.
  
  1. Educational
  
  A 50% educational workshop (classroom style) training discount is allowed for full-time students.
  
  I am a full-time student and would like to apply for the 50% off student workshop registration discount. I will produce my valid, full-time student registration card.
  
  2. Multiple Attendee Rebate
  
  A 15% rebate is allowed for two or more workshop attendee registrations from the same organization.
  
  I would like to apply for the 15% Multiple Attendee Discount. Upon completion of the workshop, your company will be reimbursed for the applicable rebate value
  
  Software Discounts
  
  Special software discounts apply when purchasing a full (non-time-limited) Single-User license of PCSWMM 2012 software along with a workshop registration.
  - 1 Day Workshop - $300 Discount
  - 2 Day Workshop - $400 Discount
  - 3 Day Workshop - $500 Discount
- Cancellation policy
- Cancellations prior to 14 calendar days before the workshop will be refunded in full. Cancellations received within 14 days but prior to 7 calendar days before the workshop will forfeit 50% of the registration fee. Cancellations received less than 7 days before the workshop will not be refunded, however registrants will be eligible for a 50% credit towards any future workshop.
  
  CHI retains the right to cancel the course up to 7 days ahead. If the course is cancelled registrants will receive a full refund of the registration fee. CHI will not be liable for any other expenses occurred by the registrant. It is sensible to make airline and other reservations that are refundable.
